You need either two witnesses or a notary — not both. If you use a notary (us), no witnesses are required. Witnesses must not be your healthcare agent, an heir, or a healthcare provider. Notarization is typically the simpler and more reliable path.

California law requires ID to be current or issued within the last 5 years. Expired ID cannot be accepted. If the patient does not have a valid ID, call us before booking — there are limited alternative identification methods available in some circumstances, such as credible witness identification.
